U.S. instructor’s failure to notice excessive reduction of engine power led to ASDF pilot’s fatal crash

On Oct. 8, the Air Self-Defense Force (ASDF) announced the results of the investigation into the training-jet crash that killed a 25-year-old first lieutenant (specially promoted to captain) in Alabama in February. The ASDF said the accident was primarily caused by the failure of the U.S. Air Force instructor, who was also aboard the jet at the time, to notice that the lieutenant, who was at the controls of the jet, had reduced engine power too much as he decelerated before landing. The ASDF was informed of the results by the U.S. Air Force, which investigated the accident.
